Plan eligibility is the first of the month following one month of full time active employment.
Medical Options -
California employees have three plan options to select from for you and your dependents. The plans available are Kaiser Permanente and an HMO and PPO through Health Net. You can choose the one that best meets the needs of you and your family.
Oregon employees have only the PPO plan available through Health Net.
Nevada employees have a choice of either an HMO or PPO plan through Anthem Blue Cross Blue Shield of Nevada.
Dental Plan - Pro-Dex has one dental plan available to you and your dependents. The plan available is a DPO through MetLife. Dental DPO is similar to medical PPO plan in terms of how benefits are provided.
Vision Care - Pro-Dex has a vision plan available to you and your dependents through Visions Service Plan.
Life Insurance and Accidental Death & Disability - Pro-Dex provides all eligible full time employees with Life and AD&D insurance on a non-contributory basis. Life insurance is provided through Mutual of Omaha. The company provides coverage equal to 100% of an employee's annual salary (minimum benefit $50K), not exceeding $500,000.
Voluntary Life - Additional coverage is available for you and your dependents.
Long-Term Disability (LTD) - Pro-Dex provides long term disability insurance for all eligible full time employees.
401(K) Plan - The company matches $0.25 for every $1 you contribute up to 5%.
Paid Time Off, "PTO" - Pro-Dex offers paid time off to eligible employees for rest, relaxation, and personal pursuits. The amount of PTO you receive each year increases with the length of your employment (26 pay periods annually). 5.7 hours per pay period.
Flexible Spending Accounts, "FSA"- Pro-Dex offers FSA's to you and your dependents for un-reimbursed medical expenses and dependent day care.
